New Patient Registration
The doctors welcome new patients who live within our practice area which for your guidance is detailed in the following table:

Please visit the surgery to complete/sign a registration form. You will also be asked to provide photographic ID and evidence of your address. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. (Alternatively you may complete the forms on-line but will still need to bring in the documents in person as soon as possible). Although you will be registered with the practice you have the right to specify a preferred practitioner.
All new patients aged 16 and over will be asked to book a New Patient Check with the practice nurse as soon as possible.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please note that we are an approved teaching practice and supervise qualified doctors who are training in general practice for placements lasting between 4 months and one year.
